Once you are invited to participate in the party, food vendors will also be required to provide the following:
All Food Vendors are required to provide a “COI” or Certificate of Insurance showing the two following entities as ‘additional insured’ on your individual policies:

Application for Temporary Food Establishment Permit-
all food vendors must obtain one prior to the night of the event and submit a copy to Kihei Fourth Friday.
